Application Forms

Please check your financial aid status to determine which forms you need to submit in order to complete your application. Prospective and current financial aid students need to check their status on a regular basis. Additional documents may be added to this list during the review process, so it is critical that you check your status frequently.

We encourage you to print, complete and submit your forms as soon as possible or by their  deadline(s). While we can accept forms that have been mailed (or scanned and e-mailed from your myCuesta email address), please note that some forms can only be accepted in person.  Please read each form carefully prior to submission.

Authorization Form

Per FERPA regulations, the financial aid office is not be able to discuss any financial aid data, status or forms with unauthorized callers.

Beginning in the 2015-16 aid year, students who wish to discuss their financial aid files via telephone or grant authorization to a third party must file a FERPA authorization form that includes a personal identification number (PIN) that will be used to authenticate their identity. 

This form may be submitted to our office in person or via e-mail from the student's myCuesta account. NOTE: Our office will NOT accept authorization forms that have been mailed, faxed, or e-mailed from a non-Cuesta e-mail account.  

 

Using the FERPA PIN

Students who call in to discuss their financial aid awards will be required to provide their FERPA PIN before we will discuss any personal information. General information--such as the application process or office hours--will be available to those who have forgotten their PIN, but no details that pertain to the student's records will be discussed.

Third parties who have been authorized to discuss a student's file with our office will be required to provide both the FERPA PIN as well as the last four digits of their social security number. They will need to provide This two factor process is intended to preserve the privacy of the student.
