We have a new health portal available to all of our Cuesta students. You may now schedule or cancel your health appointments, complete your forms and upload your documents ONLINE using your phone, laptop or tablet, and from the privacy of your own space: anytime, anywhere. Please, watch the tutorial or read the following instructions to ensure a timely process.
If you are a new hire looking for a TB test, we will respond to your email within 24 hours Monday-Friday. Contact us: studenthealthcenter@cuesta.edu. We are closed from 07/21/2023-08/13/2023. If you need a TB assessment contact the SLO public health department.

Mental Health Intakes or First-Time Appointments

- At the top, click Appointment scheduling, view appointments if you want to check your current appointments, or schedule an appointment if you want to find a new one.
- If you have a Medical Emergency or experiencing a mental health crisis, please do not use the On-line System and follow the directions on the terms of service prompt. Let's click "accept the terms of service".
- Select the Appointment Type: Mental health therapy
- Select the Reason for Appointment: Mental health Intake SLO (In-person) or Mental Health Intake Telehealth (Zoom).
- Choose the date range.
- Select the provider's name and time that works best for you and confirm.
- Wait for the confirmation message and click PENDING FORMS.
- Complete the required forms 48 hours prior to your appointment to avoid cancellation (Consent for Health Services, Crisis Prevention Cuesta College, Mental Health Intake Cuesta, HIPPA Cuesta College, and Communication Preferences).
Students may also explore the brief mental health therapy option with SLO hotline. This service is free and confidential.
Follow-Up Appointments
- Select the Reason for Appointment: Mental health Follow-Up SLO (In-person) or Mental Health Follow-Up Telehealth (Zoom).
- Select the provider's name that is serving you, time, and confirm.
- For follow-up appointments, you do not need to fill out intake forms again unless you need to update or change information, or it has been more than 1 year. Remember, forms for Mental Health appointments are different to Nursing consultation forms. Our clinic provides services for assessment, examination and simple treatment of short term, acute illness and minor uncomplicated medical problems, reduced cost antibiotic prescriptions and home care instructions. If a condition requires more than one evaluation and one follow-up visit, the patient will be referred to outside medical agencies. Click here to learn more.

TB Risk Assessment new hire- students:
This is a telehealth appointment and is available on Tuesdays and Thursdays via Zoom.
- The tb risk for a new hire is a Zoom consultation required by Human Resources and you will need to begin by completing the Release of Information form click here, allowing us to send your results to HR. This form must be completed prior to your Zoom consultation.
- After completing the release form, and receiving confirmation from Adobe sign please save a copy.
- At the top is the green banner to access the Cuesta Student Health portal, or click here: https://cuesta.studenthealthportal.com, and log in using your student credentials.
- When you access the portal, at the top click "Document Upload", for document type please select the option "Authorization to release TB Risk results to HR" and upload your saved PDF (from step 2).
- Now proceed to schedule an appointment with a nurse for a TB Risk Assessment-Telehealth using our Health Portal.
- At the top of the portal, click Appt. Scheduling, view appointments if you want to check your current appointments, or Schedule an Appoint. if you want to find a new one.

- Select the Appointment type: "Nursing consultation NCC" (please select NURSING NCC even if your main campus is SLO, otherwise you will not find appointments available for telehealth)

TB Risk Assessment as a requirement for your program/class, volunteer or work off campus:
First you need to verify with your program if you need a TB Risk Assessment, a TB Skin test, or both.
This is a telehealth appointment and is available on Tuesdays and Thursdays via Zoom.

A TB skin test requires 2 or 4 visits (for 2 steps Tb skin test) with a health care
provider.
On the first visit the test is placed; on the second visit the health care provider
reads the test.
The TB skin test is performed by injecting a small amount of fluid (called tuberculin)
into the skin on the lower part of the arm.
A person given the tuberculin skin test must return within 48 to 72 hours to have
a trained health care worker look for a reaction on the arm.
The result depends on the size of the raised, hard area or swelling.
Cost: $10 for Cuesta College Students (cash, check or charge to student account)

If it is an in-person appointment, you will also receive a notification confirming the time, location, and safety recommendations.

The student health portal will let you cancel your appointment online 24 hour hour before your meeting. How:
Log in to the portal using mycuesta credentials: https://cuesta.studenthealthportal.com
Go to appointment scheduling, view appointments, upcoming, and you will see the option
to cancel.
If you try to cancel in less than 24 hours, the system will not allow you to do it
online. Call us at 805 543317 or send an email to studenthealthcenter@cuesta.edu

the links on the left sidebar to take you to specialized information. Some links will
take you to sites outside the Cuesta College web server and Cuesta College has no
control over the content or availability of these sites.

the Cuesta Community, or Follow us on social media on Instagram/Facebook @cuestahealthcenter
for educational information, tips, advice, events and updates.
Please contact our office to request the following forms, if needed:
Telehealth services Form: Consent to receive TeleHealth services with the Student Health Center (SHC) staff and providers.
Consent for Minors Waiver: If you are a student and are under the age of 18 this waiver must be filled out prior to any physical or mental health appointments at the Health Center.
Authorization to Release or Disclose Patient Information: All appointments at the Health Center are confidential, however, if you would like to release information about your sessions and appointments to someone or an entity this waiver must be filled out.
Translator or interpreter authorization: Students may request a translator/interpreter to be present during their in-person and/or virtual sessions with health providers so that they may communicate effectively.
Acknowledgment of Confidentiality by Translator/Interpreter: Translators and interpreters must sign this form prior to the appointment to ensure that patient information remains confidential.
Appeal form for mental health therapy: Not showing up for an appointment is considered a cancellation. Less than a 24-hour notice requires filling out an appeal form. Please review our policy.
Authorization to release TB Risk results to HR: The tb risk for a new hire is a Zoom consultation required by Human Resources and you will need to begin by completing this form allowing us to send your results to HR.
NEAT Intake Forms: If you are scheduling an appointment with our Nutrition Education Action Team, please bring these forms filled out to the Health Center no later than 48 hours before your appointment.
Student Accident & Injury Report: In the event of an accident, injury or illness on campus, a student report must be submitted to the Student Health Center within 48 to 72 hours. The report should be filled out by the faculty or staff member who was advising the student during the time of the accident or injury before being turned into the Health Center. Once that report is submitted the student can begin to fill out paperwork for supplemental Student Accident Insurance at the Health Center.
Accident Reporting and Insurance: Cuesta College has limited supplemental Student Accident Insurance for injuries sustained during school-related accidents and activities. If you have other medical insurance coverage, all benefits under that policy must be fully utilized before the Student Accident Insurance policy applies. The Student Accident Insurance company will determine the eligibility of all submitted claims. It is the student’s responsibility to be evaluated, and fill out insurance paperwork at the Student Health Center within 72 hours.

Mental Health Therapy appointments are in high demand; because of this, a
24-hour notice is required to cancel an appointment. This allows us to serve
as many students as possible. Not showing up for an appointment is considered
a cancellation. Less than a 24-hour notice requires filling out an appeal
form.
Our policy is as follows:
- The 1st cancellation that is less than 24 hours will require filling out an appeal form in order to reschedule. You may reschedule at the time you turn in the appeal form.
- The 2nd cancellation that is less than 24 hours will require turning in an appeal form and waiting for the therapy staff to review and sign the form. They will decide if you may reschedule. This may take up to 2 days for approval.
- The 3rd cancellation that is less than 24 hours will terminate your ability to receive Health Center Therapy Services for the remainder of the semester. You will receive referrals to lowcost services in the community in the event that you are ineligible for services at Cuesta College.
