Mental Health Intakes or First-Time Appointments

1. At the top is the green banner to access the Cuesta Student Health portal, or in the browser type: https://cuesta.studenthealthportal.com and log in using your student credentials. You can also select the Health Portal icon when logged into MyCuesta.
2. At the top, click Appointment scheduling, view appointments if you want to check your current appointments, or schedule an appointment if you want to find a new one.
3. If you have a Medical Emergency or experiencing a mental health crisis, please do not use the On-line System and follow the directions on the terms of service prompt. Let's click "accept the terms of service".
4. Select the Appointment Type: Mental health therapy
5. Select the Reason for Appointment: Mental health Intake SLO (In-person) or Mental Health Intake Telehealth (Zoom).
6. Choose the date range starting with the day after today's date.
7. Select the provider's name and time that works best for you and confirm. 
8. Wait for the confirmation message and click PENDING FORMS.
9. Complete the required forms 48 hours prior to your appointment to avoid cancellation (Consent for Health Services, Crisis Prevention Cuesta College, Mental Health Intake Cuesta, HIPPA Cuesta College, and Communication Preferences).

Follow-Up Appointments

1. Select the Reason for Appointment: Mental health Follow-Up SLO (In-person) or Mental Health Follow-Up Telehealth (Zoom).
2. Select the provider's name that is serving you, time, and confirm. 
3. For follow-up appointments, you do not need to fill out intake forms again unless you need to update or change information, or it has been more than 1 year.

Our clinic provides assessment, examination and treatment of short term/acute illness and minor uncomplicated medical problems; reduced cost antibiotic prescriptions; over the counter meds; and home care instructions. Some conditions may require a referral to an outside provider.  Click here to learn more.

Appointments

3. Select the Appointment Type Nursing Consultation SLO (San Luis Obispo Campus) or Nursing Consultation NCC (North County Campus)
4. Select the Reason for Appointment.
5. Choose the date range beginning the day after today's date. Same day appointments need to be made by telephone: 805-546-3171.
6. Select the provider's name and time that works best for you and confirm. 
7. Wait for the confirmation message and click PENDING FORMS.
8. Complete the required forms prior to your appointment to avoid cancellation (Consent for Health Services, Health History, HIPPA Cuesta College, and Communication Preferences). 

Note: For follow-up appointments, you do not need to fill out intake forms again unless you need to update/change information, or it has been more than 1 year. Remember, forms for Mental Health appointments are different than Nursing consultation forms.

TB Risk Assessment student workers:

This is a  telehealth appointment and is available on Tuesdays and Thursdays via Zoom.

1. The tb risk for a new hire is a Zoom consultation required by Human Resources and you will need to begin by completing the Release of Information form, allowing us to send your results to HR. This form must be completed prior to your Zoom consultation.
2. After completing the release form, and receiving confirmation from Adobe sign please save a copy.
3. At the top is the green banner to access the Cuesta Student Health portal, or click here: https://cuesta.studenthealthportal.com, and log in using your student credentials.
4. When you access the portal, at the top click "Document Upload", for document type please select the option "Authorization to release TB Risk results to HR" and upload your saved PDF (from step 2).
5. Now proceed to schedule an appointment with a nurse for a TB Risk Assessment-Telehealth using our Health Portal. 
6. At the top of the portal, click Appt. Scheduling, view appointments if you want to check your current appointments, or Schedule an Appointment, if you want to find a new one.
7. If you have a Medical Emergency or experiencing a mental health crisis, please do not use the On-line System and follow the directions on the terms of service prompt.  Next click on accept the terms of service.
8. Select the Appointment type: "Nursing consultation NCC" (please select NURSING NCC even if your main campus is SLO, otherwise you will not find appointments available for telehealth)
9. Reason for Appointment: TB Risk Assessment-Telehealth.
10. Select the date range and click on the provider's name- time that works best for you and confirm. 
11. Your Zoom link will be available at the top of your health portal home page 15 minutes before your appointment. Check the section above: How to meet with your health provider at Zoom?

Cost: If you are a student enrolled in Cuesta College there is no charge ($0).

TB Risk Assessment as a requirement for your program/class, volunteer or work off campus:

First you need to verify with your program if you need a TB Risk Assessment  or a TB Skin test (PPD).

This is a  telehealth appointment and is available on Tuesdays and Thursdays via Zoom.

4. Select the appointment type "Nursing Consultation NCC" (please select NURSING NCC even if your main campus is SLO, otherwise you will not find appointments available for telehealth)

A TB skin test requires a visit with a health care provider. On the first visit the test is placed; on the second visit (48 hours after the administration of the test) the health care provider reads the test. If you need a two step you will repeat this process after your first test and read.

The TB skin test is performed by injecting a small amount of fluid under the skin on the lower part of the arm. A person given the tuberculin skin test must return within 48 to 72 hours to have a trained health care worker look for a reaction on the arm.

Cost: $10 for Cuesta College Students (cash, check or charge to student account)
 
TB/PPD tests are not able to be scheduled online. Please contact us to schedule your in-person TB skin test.

5. Reason for Appointment:  "Immunization review- Telehealth",  and select the date range.
6. Click on the provider's name and time that works best for you and confirm.

How to meet with your health provider on Zoom?

The Student Health Center is currently offering telehealth services. Your Zoom link will be available at the top of your health portal home page 15 minutes before your appointment.

To access: https://cuesta.studenthealthportal.com, and log in using your student credentials.

Click  "start your meeting now" and meet with your healthcare provider.

If it is an in-person appointment, you will also receive a notification confirming the time, location, and safety recommendations.

The student health portal will let you cancel your appointment online 24 hour hour before your meeting.

Log in to the portal using mycuesta credentials: https://cuesta.studenthealthportal.com
Go to appointment scheduling, view appointments, upcoming, and you will see the option to cancel. 
If you try to cancel in less than 24 hours, the system will not allow you to do it online. Call us at 805-546-3171 or send an email to: studenthealthcenter@cuesta.edu

Note: Mental Health Therapy appointments are in high demand, therefore, a 24-hour notice is required to cancel an appointment. Less than 24 hours notice requires the completion of an appeal form before rescheduling. This form is available on the home page of the student health portal.

Please contact our office to request the following forms, if needed:

Telehealth services Form: Consent to receive TeleHealth services with the Student Health Center (SHC) staff and providers.

Consent for Minors Waiver: If you are a student and are under the age of 18 this waiver must be filled out prior to any physical or mental health appointments at the Health Center. 

Authorization to Release or Disclose Patient Information: All appointments at the Health Center are confidential, however, if you would like to release information about your sessions and appointments to someone or an entity this waiver must be filled out.

Translator or interpreter authorization: Students may request a translator/interpreter to be present during their in-person and/or virtual sessions with health providers so that they may communicate effectively. 

Acknowledgment of Confidentiality by Translator/Interpreter: Translators and interpreters must sign this form prior to the appointment to ensure that patient information remains confidential.

Appeal form for mental health therapy: Not showing up for an appointment is considered a cancellation. Less than a 24-hour notice requires filling out an appeal form.  Please review our policy.

NEAT Intake Forms: If you are scheduling an appointment with our Nutrition Education Action Team, please bring these forms filled out to the Health Center no later than 48 hours before your appointment.  

Student Accident & Injury Report: In the event of an accident, injury or illness on campus, a student accident report must be submitted to the Student Health Center. Please fill out the student accident report right away. Afterwards, a student health representative will contact you.

Mental  Health  Therapy  appointments are in  high  demand;  because of  this,  a  24-hour  notice  is required to  cancel   an appointment.  This  allows  us  to  serve as many  students as  possible.  Not showing  up  for  an appointment  is considered  a  cancellation.  Less than  a  24-hour  notice  requires  filling  out  an  appeal form. 
Our  policy  is  as  follows:  

• The 1st  cancellation  that  is  less  than  24  hours  will  require  filling  out  an  appeal  form  in  order  to reschedule.  You  may reschedule  at  the  time  you  turn  in  the  appeal  form.
• The 2nd  cancellation  that  is  less  than  24  hours  will  require  turning  in  an  appeal  form  and waiting  for  the  therapy staff  to  review  and  sign  the  form.  They will  decide  if  you  may reschedule.  
• The 3rd  cancellation  that  is  less  than  24  hours  will terminate  your  ability  to  receive  Health Center Therapy Services  for  the  remainder of  the  semester.  You  will  receive referrals  to  lowcost  services  in  the  community.
